Prior to your appointment, please complete all documents in the Questionnaire area of your portal. Also add or update your medications, supplements, allergies, and preferred pharmacy. Please ensure your insurance information is entered and accuarate and you have added your credit card so we can collect co-pays and outstanding balances.
As of January 29, 2024, there is a modest increase in cash prices for appointments. A new patient appointment (90 minutes) is $350, a 60-minute follow-up appointment is $250, and a 30-minute follow-up appointment is $150.
